Boxing Mouthguard Rules: What USA Boxing Requires

If you're stepping into a USA Boxing ring for the first time, the mouthguard rules are short, but a few details catch boxers and parents off guard: what happens when your guard comes out mid-round, what color you can wear, and what you need if you have braces. This guide walks through what the current USA Boxing National Rulebook (effective January 1, 2026) says, and how to pick a guard that keeps you out of trouble with the referee.
Rules change, and local events can add their own requirements. Always check the current rulebook and your event's fact sheet before you compete.
Is a mouthguard required in amateur boxing?
Yes. Rule 39.1 of the USA Boxing National Rulebook is one line: "A Mouthpiece must be worn by boxers." During competitions, the rulebook also has the equipment manager check each boxer's gumshield along with the rest of their protective gear.
Professional boxing requires a mouthguard too. The American Dental Association notes that boxing is the only professional sport where mouthguards are mandated.
What happens if your mouthguard comes out
This is the rule that costs boxers points. The USA Boxing rulebook treats a lost mouthguard two different ways:
- Knocked out by a clean punch: the rulebook's quick reference lists losing your mouthguard due to a blow as a caution. If it happens a third time, you get a mandatory warning (Rule 16.1).
- Spat out on purpose: spitting out your mouthpiece without receiving a correct punch is a foul that earns a mandatory warning straight away.
Either way, a guard that pops out is more than an annoyance in competition. A loose guard is usually a fit problem, not a toughness problem. If yours already slips in sparring, read why does my mouthguard fall out? before fight day.
What color mouthguard can you wear in USA Boxing?
Under the 2026 rulebook, any color. The quick-reference chart in the rulebook states: "Mouthguards may be any color."
You may still hear coaches say red guards are banned. An older USA Boxing uniform guideline sheet said no red, pink or orange mouthguards (or any mix of those colors), so that advice was once right. If your event's fact sheet or the officials at your show still apply a color limit, follow them. If you want to avoid any argument at check-in, a clear, white, black, blue or green guard is the simple choice.
Origin and Origin Pro come in base colors including Clear, White, Black, Green, Red, Blue and Yellow, or two or more colors.
Boxing with braces
You can box with braces in USA Boxing, but there is paperwork. Rule 39.2 says boxers who want to compete with braces must attach a completed Release to Compete with Braces form (Appendix G of the rulebook) to their passbook. If the boxer is under 18, a parent or guardian has to approve it in writing. Boxers competing with braces also waive dental coverage under the USA Boxing insurance program.
The form itself includes an agreement to wear a molded mouthpiece while boxing. Athletes with braces can get AlienGuards, but your dentist needs to take the impression instead of using our at-home kit, so your braces don't get damaged. Your dentist can take an impression or 3D scan, then ship it to us or email it to impressions@alienguards.com. More in can you wear a mouthguard with braces?
What to look for in a competition boxing mouthguard
The rulebook only requires that you wear one. What makes a good one is up to you, and in boxing it comes down to three things:
- It stays in. A guard that has to be clenched in place is a guard that can be knocked out. A custom guard made from an impression of your teeth doesn't slip or fall out the way a loose guard can.
- It handles direct hits. Our combat guards are 5 mm, the industry standard for competitive combat sports. Read how thick should a mouthguard be? for how 4, 5 and 6 mm compare.
- You can breathe through a full round. Custom mouthguards are thin and precise, so most athletes breathe and talk normally.

Origin or Origin Pro for boxing
Origin is our combat guard for boxing, MMA, Muay Thai and kickboxing, with reinforced impact zones and 5 mm thickness. It starts at a lower price and is the only model with the Trim for Gag Reflex option.
Origin Pro has the same 5 mm protection with more breathability and comfort, lets you put a logo and text together, and offers an optional High Impact 6mm upgrade for super high-impact training and fights.
For a side-by-side look, see our custom boxing mouthguards guide. If you also train MMA, Origin and Origin Pro cover that too.
Spar in the guard you'll fight in
Fight night is the wrong time to try a new mouthguard. Wear your competition guard in sparring so you know it stays put, you can breathe in it, and you can hear your corner. Our guide to the best mouthguard for sparring covers what to look for in training.
